Our clinic is owned and operated by a Board Certified Nurse Practitioner who provides customized primary care where she takes the time to know you, understand your goals and partner with you to stay healthy-today and long term.

Self-pay, Uninsured Fees

A Self-pay patient is either uninsured or has an insurance plan that is not in-network. Self-pay patients pay in full at the time of visit for services provided. The clinic is not required to file a claim or submit any documentation to a third party. These rates are not offered to patients covered by insurance plans with which we are in-network. We are required to file claims when a patient has health insurance in which we are in-network. **Rates are provided upon request.
